Service Coordination For Children and Families

What is Creative Options?
Creative Options helps to bring children and families and the services they need
together, in a way that is simple and organized. Creative Options can improve
results for children and their families.
Who is eligible?
Children and their families with multiple needs from birth to age 18. Needs can
be behavioral, emotional and/or physical. Needs may be identified by a child,
family, school or counselor.
Creative Options Plans are developed by a:
*Family
Team (including the parent(s))
Based upon:
*Family strengths
*Family wants & needs
*Agency recommendations
*Setting where services can be most helpful to the
child
Service Coordination Plans can include:
*Services to be provided
*How & who will pay
*Ways to deal with a child, including ways to keep
the child out of the juvenile justice system
*When the plan starts & stops
